Is that really Megan Fox? The "unrecognizable" clip that has fans crying surgery

A year-old clip of Megan Fox talking about her favourite movies landed this month, and the comments got so wild the account switched them off. The truth behind it is more interesting than the rumours.

"What happened to her face?"

That was the question flooding the comments under a cheerful little movie video this month. Megan Fox, sitting next to director Karyn Kusama, naming the films she loves most. Braveheart. The Lion King. Teenage Mutant Ninja Turtles II. The Harry Potter series.

Nobody was talking about the movies.

Within hours, the comments got so heated that Letterboxd, the film site that posted the clip, switched off comments on its Instagram post.

Where is the "unrecognizable" Megan Fox video from?

The clip is part of Letterboxd's "Four Favorites" series, where stars name the films they would take to a desert island. Letterboxd released it on October 1, 2026.

It was filmed almost a year earlier. On October 25, 2025, Fox and Kusama sat down at the Academy Museum of Motion Pictures in Los Angeles for a special screening of Jennifer's Body, the 2009 horror comedy that bombed on release and later became a cult favourite.

Fox arrived that night in a sheer tulle dress beaded to look like it was dripping blood. It was one of her first big public appearances after having a baby. Kusama told the crowd that writer Diablo Cody has ideas for a sequel.

None of that made it into the comment section.

What are people saying about Megan Fox's face?

Fan pages reposted the clip, and it spread fast. People put it next to photos of Fox from her Transformers days in 2007 and asked whether she had had more work done.

The same thing happened in August, when Fox posted a glam video of herself reciting lines from Jennifer's Body. "Megan Fox does not look like Megan Fox," one person wrote. Another said they thought it was a random model trying to look like her.

Some went further. Earlier this year, one commenter said she was starting to look "completely like AI". Others simply wrote that she was always naturally gorgeous and didn't need to change anything.

None of these commenters know what, if anything, she has had done since 2024. It is speculation, and loud speculation at that.

Why is Megan Fox in an erectile dysfunction ad?

The timing made everything louder. On the same day the Letterboxd clip dropped, Fox launched a new ad campaign for Ro Sparks, a prescription erectile dysfunction pill that combines sildenafil and tadalafil, the active ingredients in Viagra and Cialis.

In the ad, Fox plays the pill itself. She wears a skin-tight red catsuit with the logo on it and promises to show up at your door in very discreet packaging.

She told People she felt she was "born for this". Ro says the point is to get people talking about a condition many men hide.

The internet talked, all right. Under the ad, comments compared her to Madonna and said she "doesn't even look like her". Two viral moments on one day sent "Megan Fox unrecognizable" trending on Google.

What plastic surgery has Megan Fox actually had?

Here is the twist. Fox has admitted to more than most stars ever do, and fans say the list doesn't explain what they are seeing now.

On the Call Her Daddy podcast in March 2024, she went through her history in detail. She said she had her breasts done at 21 or 22, again after she finished breastfeeding, and once more recently. That makes three breast surgeries in total.

She said she had one nose job in her early twenties and none since she was about 23. People had accused her of six, seven or eight rhinoplasties, she said, which is impossible because the nose would "get necrosis and fall off". The slim look in photos, she explained, comes from heavy contouring.

She also admitted to laser treatments, Botox and filler. She ruled out buccal fat removal and liposuction, saying she would only ever put fat in and never take it out.

Her reason for sharing it all was simple. She said she hoped it would set some people free.

That was two and a half years ago. Fans comparing the old clips with the new ones are asking the obvious question. What has happened since?

Has Megan Fox talked about body dysmorphia?

Yes. In her 2023 Sports Illustrated Swimsuit cover interview, Fox said she has body dysmorphia, a condition that makes people fixate on perceived flaws in how they look.

"I don't ever see myself the way other people see me," she said. She added that there has never been a point in her life when she loved her body.

That interview gets dragged into every new round of face speculation. Some fans use it to argue she has gone too far. Others say it is exactly why strangers should stop picking her face apart in public.

What do plastic surgeons say about Megan Fox?

Surgeons have weighed in before, without ever treating her. Earlier this year, two board-certified plastic surgeons told DailyMail.com what they thought they could see in her photos.

Dr Norman Rowe said her nose bridge and tip had changed and that she appeared to have Botox in her brows plus lip and cheek filler. Dr Charles Pierce floated the idea of an endoscopic brow lift.

Neither has examined her. A brow lift would be new, and it is not on the list Fox shared. She has never said whether it is true.

Why are fans defending Megan Fox?

The backlash to the backlash arrived almost as fast as the comments.

The biggest point fans keep making is about timing. Fox's daughter Saga was born on March 27, 2025. The screening was seven months after she gave birth to her fourth child. Defenders say a face and body changing after pregnancy is normal and no reason for a pile-on.

Others point to lighting, camera quality and styling. Fox made the same argument herself in February 2024, when a dark phone photo from a Super Bowl party in Las Vegas set off the same rumours. She posted a brighter shot from that night and blamed the "shadowy cell phone pic".

Some fans also say that comparing a woman of 40 to photos of herself at 21 says more about the comparer than about her.

Has Megan Fox responded to the comments?

As of October 7, Fox has not commented on the Letterboxd clip, and the story is still developing.

She rarely stays quiet for long, though. In March, someone commented on one of her Instagram posts that it wasn't really Megan but a clone. Fox wrote back that a clone could never.

In July, a commenter called her photos "unbelievably embarrassing for a 40 year old mom to be posting". Her reply went viral. "Which one of my exes is this," she wrote.

So the question hangs there. A new procedure, a new baby, bad lighting, or all three? Fox has not said, and the internet has already made up its mind.

The one thing nobody disputes is that she always gets the last word. Until she talks, millions of strangers will keep zooming in on that face, and she will keep winning every comment fight she picks.
